Zoneee

Click "Settings"

Click "Appointment Types"

Click on the appointment - "New Client - Chiropractic"

Click on "Form Template to fill in by Patient"

Select the form you want to attach

Click "Save"

Click "Notifications"

Click this icon to open pop up

Select "Confirmation with form" this means the customer will get conformation of their appointment and the form will be attached

Click Icon to "Preview and Edit"

Click "Edit"

Edit as necessary

You can use additional placeholders by clicking this icon

There is different placeholders such as "Clinic Name" "Clinic Phone" "Customer Name"

Once done - Click "Save Changes"

Click "Yes" & you are done
