Requesting SLOA

Navigate to [https://graduate.ucf.edu/forms-and-references/](https://graduate.ucf.edu/forms-and-references/)

Click "Special Leave of Absence"

Click "Complete This Form"

Enter student - First Name - Last Name - UCF ID - UCF email address

Respond to question: Are you an international student?

Respond to question: Are you pursuing a graduate degree program (e.g. master's, specialist, or doctoral degree?)

Select program College from the dropdown menu.

Respond to question: Are you pursuing a graduate certificate?

Indicate agreement with the SLOA policies. Click "Yes"

Select the term (Spring, Summer, Fall) and enter the Year for each semester of requested leave.

Enter the Term/Year to return to UCF.

Enter the earliest admit term (Term/Year)

Upload your Unofficial Transript

Respond to the question: Have you been enrolled in your graduate program for 5+ years?

Enter your explanation for requesting special leave. Be a detailed as possible.

Attach any supporting documentation or files.

Respond to the question: Would you like to upload additional documents?

Respond to the question: Are you a Graduate Program Director or staff member submitting this on behalf of a graduate student?

Click to enter Signature

Enter date.

Click "Submit Form."

You'll be notified of your request decision or if any additional information is needed via email from gradservices@ucf.edu.

Returning from SLOA

When returning from SLOA, if you have a registration hold that is preventing you from enrolling in your approved return term, reach out to the Student Services Team in the College of Graduate Studies for assistance: - Email: [gradservices@ucf.edu](mailto:gradservices@ucf.edu) - Phone: 407-823-2766, select option 3 for student services
