Starting Off, If you were requested to submit one by someone else, Click here and answer "Yes" if not, then answer "No"\ \ If you Answer "Yes" you will be prompted to enter YOUR Information\ \ Then you will enter THEIR Information later in this section. (Refer to step 4 - 5 as this is where you'll enter THEIR Info)

Click "Select Which type(s) of billing this is for all services that will be entered Required"

Click "Select Which type(s) of billing this is for all services that will be entered"\ \ New Account = New account starting/never serviced this account prior\ \ Extra Billing = Extra cost to customer that is outside of "Normal" Costs\ \ Subcontractor Pay = Contractor is getting paid\ \ Revision to a Current Account = Revision to a previous standing BS/SA\ \ Subcontractor Change = Subcontractor is being changed from the current one

Click "Your Name: (Submitter/Requesting Person IF filling out for someone else)"\ \ If you answered "Yes" to "If you were requested to submit one by someone else" (Refer to step Step 1)\ You will enter THEIR Info here\ \ If you answered "No" you will enter YOUR Info Here

Click "Your Email: (Submitter/Requesting Person IF filling out for someone else)"\ \ If you answered "Yes" to "If you were requested to submit one by someone else" (Refer to step Step 1)\ You will enter THEIR Info here\ \ If you answered "No" you will enter YOUR Info Here

Click "Account Name Required"\ Name of the account this is regarding

Click here to specify the certain area IF it is only a certain area.\ For Example\ \ "Account" = "KGFS Office" however we are only servicing the "Warehouse Area, not offices."

Click "Yes"

Click "Area/ Building Name:"

Click "Address for the Account/Building"\ Enter the address of the account, if it auto populates click the auto populated address, if it can not find the address, you will have to enter it manually.

Click the auto populated address if it finds it.

Click "Bill To:" to enter who is going to be paying for the service, it usually is the same as the account name.

Click "<https://analytics.zoho.com/open-view/2723708000002635073>" to find our current active subcontractor contact info list.\ please copy the info and do not manually type in info, as this can cause delays / issues during the process.

Click the tab where the form is to go back to the form.

Click "Yes" if the service will be repeated exactly on a recurring basis.

Click "No" if the service will be NOT repeated exactly on a recurring basis.

if you selected YES to the previous Question\ \ Click here to select yes or no if the contractor you entered now is going to replace the contractor that is currently in place.\ if it is the same contractor , please select No\ \ Example: Different \ Current Contractor in the account is Contractor A\ The contractor we are entering is Contractor B\ We would select YES in this case\ \ Example: Same\ Current Contractor in the account is Contractor A\ The contractor we are entering is Contractor A\ We would select NO in this case

Click "Next" to Navigate to the next page

Click "Effective Start Date:"\ \ this is the effective start date of the service.

Select the date

Click here to say yes or no if there is a effective end date, however depending on the service, you may only be able to select YES or NO

In this example we can only select YES

Click "Effective End date:"

enter the end date for this service

Click here to enter the Effective HOUR service Start time for the contractor to start the service

Click the hour you need

Click here to enter the Effective MINUTE service Start time for the contractor to start the service

Click "Service Agreement Amount: $"\ \ this is how much the contractor is getting paid for the service

Click "Invoice Amount: (what we charge the customer/client)"\ this is how much we (KGFS) are charging / invoicing the customer (Account) for this service

Click here\ if there is any material costs or other outside costs that KGFS is paying for , for this service\ \ Examples:\ Scissor lift rental\ Machine rental

Click "Material/Supply/extra costs to KGFS for this service: (total) $"\ to enter the cost of said "materials/extra costs"

Click "Description/Comments for this service:"\ \ This is where youll be able to put any and all information pertaining to the service, account, or info that the contractor needs to preform service.\ Please DO NOT include any duplicate info that you have entered within the form\ you DO NOT need to enter days of the week, start/end time/ account name/pay to contractor/address/ etc\ \ An example of info to put is door codes, alarm codes, key codes, where janitorial closet is, specifics about the service being preformed, how to enter the account, referring to scopes of work, etc

Click "Next" to go to the next page

depending on the type of billing type, you may have to enter internal notes.

Click "Internal Notes (only shows on billing sheet) "\ \ internal notes is any information you need to put that only KGFS can see\ The contractor can not see this information.

Click "Yes" if youre receiving commission for this BS/SA Submission

Click here if you are receiving a custom % commission

Click "Yes"

Click "For Custom Only: Percentage of Commission for you %" \ this is where youll enter the custom %

Click here if it is dual commission (two people are getting commission)

Click "No" if there is not 2 people getting commission

Click "Who is authorizing your commission(s)? Required"\ this is where you'll enter the name(s) of who is authorizing your commissions and %

Click here if you are voiding a previous BS/SA submission, this also means that this current submission is superseding / replacing the previous Submission

Click "Yes" if you are voiding a previous BS/SA submission, this also means that this current submission is superseding / replacing the previous Submission

Click here to select how many you need to void

Click "1 - 3"

Click "What is the first SA Number you need to void? "\ Enter the BS/SA # you are voiding/Superseding with your current submission
