How To: Upload Concussion Medical Clearance Certificate

**Open your credentials**\ In your profile, select **Profile**, then select **Credentials** to open the credentials tab.

Click **Add Credentials**

**Search for a credential**\ Type a **keyword** into the search field to find the credential you wish to upload.\ For example, type **concussion** to search for the **Concussion Medical Clearance** credential. *Note:* You can narrow your search by changing the **credential category**. The Concussion Medical Certificate is under the category **Safeguarding**

**Select the credential**\ Click **Concussion Medical Clearance** to open the credential.

**Upload your medical clearance**\ Click **Upload** to attach your medical clearance document.\ *Note:* You cannot change the dates, as these record the date the Medical Clearance form is submitted to EA for review, so the suspension can be lifted. Select **Save** to save your progress and upload the credential to your profile.

**Pending approval**\ Your **Concussion Medical Clearance** will show as *Pending Approval* until it has been reviewed and approved by a member of the **EA Health and Safety Team**.\ \ When approved the credential will appear with a green flag and the status will change to Active. \ \ Until both the minimum stand-down time (i.e. 21 days) and the approval of the Medical Clearance form is granted, your **concussion suspension will remain in place**.
