How do I add an additional clinic?

Navigate to [https://hs.freshclinics.com/additional-clinic#form](https://hs.freshclinics.com/additional-clinic#form)

Fill in your additional clinic details here

You will need to select if this is a residential clinic
Alert! Please make sure you have spoken to a member of Fresh Clinics before registering a residential address to ensure it will meet compliance.

Click the "ABN Certificate" field to upload your ABN Certificate

Click the "Poison Certificate" field to upload your Poison Permit (For VIC + WA only)

Fill in your opening hours

Click the "Choose files" field to upload the required photos

Click the "Emergency Medication" field to upload your emergency medication expiry dates

Select if you will be receiving S4 deliveries to this clinic

Click "submit"

Click "Record your compliance video now"

Listen to the instructions and Click "Next"

Click "I'm ready to record"

Click "Record your answer"
