How do I build a consent form for my patient?

Firstly login to your Fresh Clinics App on your device.

Select your patient record.\ To add a new patient Click "new patient" at the bottom right.

Click "New Treatment".

Click "Add" next to consent form.

Select the type of consent form you would like your patient to complete.\ If your patient is being treated with Filler and Anti wrinkle you will need to ensure you select both.

Once you have selected the type of consent form Click "Create consent form".

Now ensure your patient reads and answers all questions.

Now have your patient confirm their email address and Click "Tap to sign".

Once your patient has completed their signature click on the green tick.

Click "I confirm this is true".
