How do I complete a questionnaire sent by my professional?

You will receive the questionnaire by email from your professional. If you don’t see it in your inbox, be sure to check your spam folder. Your professional can only send it after you’ve given permission, are linked, and have an account.

Open the email and click "Complete the questionnaire".

Read the introduction on the first page. Take your time to answer all questions honestly and calmly. It might take a while, but the more accurate your answers, the better the outcome. Your progress is saved automatically. All questions must be answered, even if the answer is 0. Then click 'Start'.

Fill in the questions from the first section. You can only continue after answering all questions on the page.

Click "Save and continue to next page" to proceed.

Answer the questions from the second section. Again, all questions are mandatory.

Click "Save and submit".

Complete the third section entirely.

Click "Save and submit"

The number of sections may vary per person. This is determined by your professional based on the information they need. Your questionnaire may therefore contain more than three sections.

Click "Submit"

Once everything is filled out, a confirmation page will appear: "Thank you for completing the questionnaire. Your answers have been successfully submitted and will be reviewed shortly." They will be discussed during your next appointment.
