How do I create a personalised care plan for my patient?

Go to [https://supplentia.com/be/en](https://supplentia.com/en/en)You'll land on our homepage.

Click “For professionals” in the main menu.
Log in with your professional account. Don’t have one yet? Create an account first.

Click “Care Plan” in the menu.

Click “New Care Plan” to start creating a new one.

Click "Add patient"

Start typing the patient’s name or email address.

Click “Add new patient” if the person isn’t yet in your account.

Select the correct patient from the list.

Double-check the selected patient is correct.

Click “Select” to link the patient to the care plan.

Click “Add products” to open the product catalog.

Click “Add” next to the desired supplement. Repeat if you want to add multiple products.

Click “Remove” to delete a product from the plan if needed.

Click the cross icon in the top right to exit the catalog.

Click “Next” to proceed.

Choose an option depending on your preference.

**Send an overview of the care plan**\ The patient receives a link to view the recommendations and place the order themselves.

**Place the order for your patient**\ The patient receives an invoice by email and the products are delivered to their home.
