How to Add New Diagnosis to Patient Medical History?

To add a diagnosis, go to the selected patient’s profile and select "Medical history" tab on the left.

Click "+" icon in the right top corner

Add "Diagnosis"

You can search using the ICD 10 dictionary

You have the option to add a "Description"

The Year field is required. If you do not know the exact "Start date" of diagnosis, you do not need to fill it in, you can add a comment instead.

Indicate whether the diagnosis is still ongoing or not

Add "Severity" from the drop-down list.

Indicate whether any Concomitant medications have been administered for this diagnosis.

Click "Save"

After adding the diagnosis, it will appear in the “Medical history” list. Clicking on the three dots next to this diagnosis will bring up the possible options for action.

You can "Open", "Edit" and apply changes, "Delete" and check Version history for this particular diagnosis.
