Enter your name and student email

Enter your internship site supervisor's name and email

Enter your faculty sponsor's name and email - this is a professor in your field (prior permission from them is needed)

ONLY enter fields if you are an international student: use "international@northpark.edu" as the email

Click "Begin Signing"

Click "GOT IT"

Click "I agree to use electronic records and signatures. Required"

Click "CONTINUE"

Select the term for which you will complete the internship. Note that you need to submit a NEW application if you are doing an internship for more than one term.

If you are doing a full semester internship do NOT click on "Quad A" or "Quad B"

Complete ALL required fields

Enter the number of credits you are requesting. NOTE: You can add 1-8 credit hours, but double check with your faculty sponsor to identify whether your major requires a certain number of credits

Every credit hour that you request = 40 hours at the internship site (1 = 40; 2 = 80; 3=120, etc.).

Repeat information above

Type your name and sign

Click "ADOPT AND SIGN"

Select correct term and quad if it applies

Enter required fields

Type "X" to indicate that you are adding

Click "Sign"

Click "FINISH"

Do not resubmit your application - this screen indicates that you have completed your part of the application and has now moved forward to the next signee.

NOTE: Once you sign, your application automatically gets sent to your employer/site supervisor. Once they sign, it gets sent to your faculty sponsor and so forth. You DO NOT need to add or plan this class on Self-Service. The last office to get your application is the Office of the registrar who is responsible for adding the internship to your schedule.
