How to Add the Physical Form and Date of Physical to a Report

Learn how to efficiently modify your organization's program settings by adding specific fields to a report. This guide walks you through the configuration steps to ensure your student records remain accurate and up-to-date.

Program Report Access

Navigate to your "Programs"

Click the numeric count in the "Filled" column for the program you wish to view.

Select the "Links" dropdown menu

Report Configuration

Click "Edit" in the dropdown

Scroll to the "Choose Fields" Section

Scroll down to "Custom Fields" and click the dropdown

Click "2026-27 Physical Exam (Section 6)" or whatever you named your physical form

Once you click a field, you'll notice it on the right hand side in the report.

If you added "Date of Physical" to your "Internal Fields" click on the "Internal Fields" dropdown

Click "Date of Physical"

Click "Save & View"

Click "Save Changes"

View Optimization

Click the "Columns" dropdown

Select all the Fields you want to view in your report

Click "Apply"

View your Report
