Navigate to [https://sonia.nl.edu/School.aspx](https://sonia.nl.edu/School.aspx)

Click "Forms"

Scroll down to the lower section labeled "Students". Find the assessment form to complete.

Click the paper icon on the right side of the corresponding row to access the form.

Add your comments in the box, and click "Mentor Teacher Submit".
Tip! You may save this form as a draft and return to submit it. Simply click the "Save Draft" button to do so.

To verify the completion status of each form, view the "Completed (Mine)" column.
Tip! The (Completed Mine) column refers to you, the mentor. The (Completed All) column refers to you, the teacher candidate, the coach, and OTD.
