Navigate to [https://graduate.ucf.edu/forms-and-references/](https://graduate.ucf.edu/forms-and-references/)

Click "Administrative Record Change - Program Staff Request"

Log In using NID email and NID Password. This will auto-populate your information into the form as the initiator.

Click "Complete This Form"

Enter the Student's Information: - First Name - Last Name - UCF ID - Email Address Click this text field.

Select College from the drop down menu.

Enter the Graduate Program Name for the student.

Respond Yes or No to "Are you an office of Experiential Learning faculty member submitting a Curricular Practicum Training (CPT) request?"

Enter the information for the College Associate Dean or designee to sign-off on the form: - First Name - Last Name - Email Address (this is how the form will be routed to them) Click this text field.

Select the Administrative Record Type from the drop down menu: 1. Late Add 2. Late Drop 3. Late Withdrawal

Enter the Term. Format: Semester Year (Fall 2023)

Acknowledge the statement regarding accuracy and processing time.

Select the action for each individual course for the request.

Enter the course information paying special attention that the section, course number, and instructor match. - Course Prefix and Number - Course Section - Course Credits - Instructor First Name - Instructor Last Name

Enter a detailed explanation for the administrative record change request.
Keep in mind the scenarios and time frame for processing. - Is it in the student's best interest for success at this time? - Is the student merely dissatisfied with a course or graduate program? This is not sufficient grounds for a late withdrawal or drop. - Did the student experience a serious injury to an immediate family member or a mandated and unavoidable employment change? This is sufficient grounds for a late withdrawal.

Attach any appropriate documentation such as emails, syllabi, etc. Verify your information in the "Initiator Information" section.
Tip! Late Adds require approval from the instructor. Also, make sure sections are not closed.
