How to Create a COI/C or OE Request (Employee/Initiator)

Annual Notice to Employees Inbox

For employees who have the inbox notice to complete the COI/C Submittal, please navigate to your "My Tasks Inbox".

The inbox item is a notice, it is not the Conflict of Interest/Commitment questionnaire. Clicking submit on the inbox notice is not completion of the required task.

To complete the COI/C questionnaire, employees MUST click the orange "Create Request" button. This will take you to the drop down to select the "Conflict of Interest & Commitment |UAF" form. The questionnaire questions must be answered and submitted.

Click the "Request Type" field.

Type "conflict [[enter]]"

Click "OK"

Manual Submission of the COI/C Disclosure

Click the "Search" field.

Type "create request [[enter]]"

Click "Create Request"

Click the "Request Type" field.

Type "conflict [[enter]]"

When completing the Conflict of Interest/Commitment (COI/C) Form, if you select that you have outside employment, a 'to do' will populate when your COI/C form is closed by UAF Research Compliance. If you are not prepared to complete the Outside Employment Form, you can submit the 'to do' and initiate Create Request at a later point in time.

The request form for "Conflict of Interest & Commitment | UAF" will auto populate. For Outside Employment, type UAF and select Outside Employment - UAF.

For this example, COI/C is selected. Once the form is selected, click "OK"

Completing the Form(s)

Each form (COI/C & Outside Employment) has specific questions that employees should be prepared to answer.

Click "Yes","No", or provide an attachment appropriately, based on the questions asked. For questions about disclosures please refer to Fayetteville Policies and Procedures 404.0: <https://vcfa.uark.edu/fayetteville-policies-procedures/vcri/4040.php>

If you have selected that you have or potentially have a conflict of interest/commitment, click the "Yes" field.

You may be required to attach a Management Plan Form or a detailed explanation of the COI/C. Click "Select files" to upload an attachment.

If you are engaged in outside employment, Click "Yes" and a new inbox item will be distributed to your "My Tasks" to complete the Outside Employment Form.

Click "Submit"
