How to Fill CORI Request Form

These instructions explain how to fill out the CORI background check request form for LPS volunteers. You MUST GO INTO THE SCHOOL and verify your ID. LPS needs five business days from when you verify your ID to process the CORI. A CORI background check is valid for three years.
