Navigate to [Civil Air Patrol eServices](https://www.capnhq.gov/eServices.aspx)

Click "Menu"

Click "Logistics"

Click "Operational Resource Management System (O.R.M.S)"

Click "Report of Survey" for the item you wish to process.

Review the recommended assessment from the Wing CC.

Click "View File" to review the ROS Checklist.

Review the Member(s)/Unit to be Assessed. \*\*\*If you **do not agree** that this is the correct member to be assessed, click "Delete Member from Assessment".

If you wish to assess an **additional** or **different** member or unit, search for the member to be assessed. \*\*\*If you do not wish to assess a member/unit skip to step 13 and label the assessment a Type **NONE**. OR If you agree with the Wing CC's addition of member/unit, skip to step 13.

Select which member is to be assessed from the list.

Next, you will enter **$0** in the "Amount to be Assessed ($)" box. **ALWAYS enter $0 here.**

Click "Add Member with Amount" button.

Enter **$0** in the "\*Unit Share of Assessment" box, and **$0** in "\*Total Member(s) Assessment" box. **ALWAYS enter $0 here.**

Select the "Type of Incident".

Input your assessment recommendation in the "\*Region Commander's Comment (Concur/Non-Concur)" box. Please be descriptive as to what the assessment will be, ie. documented counseling, retraining, suspension, or termination of membership in the most egregious cases.

Click "Submit to NHQ" button.
