How to Submit a Service Request

Click "Service Request"

Click this dropdown. Choose your provider.

Click this dropdown. Choose your recipient.

Click the "Service Start(Required)" field. Choose your first or only date of service.

Click the "Service Duration(Required)" field. Enter the number of hours provided by the provider for the only day or all the days combined in the claim.

Click the "Total Cost(Required)" field. Enter the amount paid to the provider for the claim. Must match the uploaded receipt.

Click the "Single Receipt File" field. Choose the file on your device that contains your receipt.

Click "Select files" You may upload multiple files if necessary to show the amount paid to the provider.

Have your provider sign for the date(s) of service.

You must add in the comments section all the dates of service provided by the provider.

Click "Submit" to complete your claim.
