How to Submit an IBC Protocol Online

Cayuse

Navigate to [https://illinoisstate.app.cayuse.com](https://ilsu-t.uat.cayuse.com/) If you aren't logged in automatically, log in using your ILSTU ULID and password.

Click on the Products dropdown to select the software you are looking for.

This dropdown will display Cayuse Products that you have access to. Hazard Safety is the protocol submission product for the Institutional Biosafety Committee. If you have an IACUC protocol, you may see Animal Oversight. It is important to make sure that you are in Hazard Safety when submitting a biosafety protocol.

Select the appropriate role using the Role selector dropdown.

This dropdown will display all of the roles that you have access to. If you do not see the role listed that you need (most will only need "Researcher Staff Member" role), reach out to [ISUbiosafety@ilstu.edu](mailto:ISUbiosafety@ilstu.edu).

To start a new protocol, click "Start a New IBC Application"

Click "Start a new IBC Protocol"

Click the appropriate fields based on research and teaching activities covered by the biosafety protocol.

Once all applicable fields have been selected, click "Next page"

Throughout the form, purple question marks provide hints and additional information to complete the form.

Start filling out the forms in order. The Table of Contents will only display forms that match selections on the previous page.

This is a grid page. In this example, to add content, you must select "Add Funding Source"

On all pages, you must click "Save changes" in order for a green checkmark to appear next to the form. All required fields marked with an asterisk must be completed before the form can be saved.

On the Accidental Spill/Splash Response page, all three tabs must be completed before the form can be saved.

Once you have completed all required fields and have green checkmarks in all sections on the left, click "Submit Protocol".

Click "Submit Protocol"

Your submitted protocol will now appear in the "Protocols in Review" section.
