How to create a Influenza Vaccination Consent Form

Navigate to your [HotDoc Dashboard](https://www.hotdoc.com.au/dashboard#/), then click "**Vaccinations**" then click "**Flu**"

Under the appointment setup, click "**Create Or Link Form**" **Note**: If you have not completed the flu vaccines set up, please complete this and you can enable the consent form during this setup. You will see the icon change to a green tick ✅. Click "**Edit**" to view your form to make any changes or make this live.

You can select the 'Edit' button to make any changes to the questions, alternatively to make this form sent to patients click "**Make live**"

You will then see the appointment types that we will send the form for and some additional information. \ Click "**Yes, make live**" and we will begin sending this to patients once they book an appointment type that is linked to this form.

**Please note:** By default, if the received and submitted a flu vaccine form **less than 6 months ago**, we **will not** send another form. \ \ However, you can edit this for your form in the **Forms** settings by going to **Edit > Settings** under '**How often would you like patients to fill in this form?**
