How to request a permit (Staff)

Example used is Hot Works Permit. Details of documents you need for all permits or certificates also included in this guide.

Warwick

Navigate to [Portfolio - Explorer - Invida](https://estates.resolve.warwick.ac.uk/app/portfolio/explorer)

Click "Task Management"

Click "Works Authorisations and Permits"

Click "Permit/Certification Requests"

Click "New Request"

Choose the Permit or Certification you desire

Click the "Company/Organisation" field.

Type in the team or Service Provider this Permit is for

Choose activity Start date

Choose the correct Start Date

Type in Activity Start time

Select Activity End Date and time

Type in your Order/Job number (if applicable)

Type in Description in Works the work taking place: For example "Welding of steel pipework and brackets within the plant room to support mechanical services installation"

Type in Details relating to the job. For example: "Electric arc welding will be carried out on pre-installed steel pipework and associated supports."

Type in Name and Contact number of fire watch person

Type in Fire extinguisher training date

Fire extinguisher training evidence/certificate must be uploaded in the **Files** section

Type in Location of smoke or heat detectors close to work area

Type in number of sensors or detector caps required

Choose Yes or No to whether this permit is required for weekend working

Add in any additional comments if required

Type in **Name**, **Email** and **Phone number** of person who is completing the request form (permit requestor)

Choose the location the work will be carried out in

Click the "Enter search terms..." field to find the space you are after

Tick the room or rooms this Permit is for.

Click "OK"

Upload your Risk Assessment document

Upload your Method Statement document

**Key documents that must be uploaded for individual requests** You must ensure that the appropriate documentation is uploaded prior to submitting your request:

Type in the name and phone number of the person(s) carrying out the work (minimum of 2 people).

Click away from the box

Scroll to the top and Submit Request

Tip! If any mandatory fields have not been filled in, the system will notify you.

The Permit will appear in the list of Permits you've created. To review the Permit, click on it.

You can review the Permit you've created and either Cancel the Request; Add a Comment or Copy it.

Tip! The Permit will now be reviewed by the Admin team and the Permit team for approval.
