How to set up a Zap to send Assembly (formerly Copilot) forms every month to a client or clients

Zapier
As of September 30, 2025, we’ve rebranded to Assembly. The steps below remain the same. When searching in Zapier, look for Assembly (formerly Copilot) and this logo.

Go to Zapier and create a new Zap

Click here.

Click the "Schedule" trigger from Zapier

Click the "*EventRequired" field.

Click "Every Month"

Click "Continue"

Click "Choose..."

Select the day of the month you wish to request the form

Click "Choose..."

Select the time of day you wish to request the form

Click "Continue"

Click "Test trigger"

Click "Continue with selected record"

A new modal will appear to allow you to select the next action. Type "copilot"

Click "Copilot"

Click the "*EventRequired" field.

Click "Request Form Responses"

Click "Continue"

Click "Choose" to select your Copilot account, or connect a new one

Click "Continue"

Go to the Action section and click "Choose value…" to select the recipient of the form.

Select the client you wish to share the form with. You can share forms with multiple clients as needed in the input that appears after selecting the first client

Click "Choose value…"

Select the form you wish to share monthly
