How to submit a Grandview Kids eReferral (for physicians)

Below is a series of step-by-step instructions, walking you through how to complete a Grandview Kids eReferral via Ocean. If you have questions after reviewing these instructions, please email Contact@ereferralontarioeast.ca.

To submit an eReferral, you MUST create an Ocean account AND site. If you haven't yet created either, please visit <https://www.cognisantmd.com/getting-started/> and watch the brief instructional videos in Steps 1 and 2 to get you set up.

Navigate to the Ocean website\ Type "Grandview Kids" in the search bar; type "Durham Region" into the adjacent location field.\ Click "Any listing containing: Grandview Kids."\ Click "Grandview Kids - Client Services (for Healthcare Providers)"

Click "Send eReferral"

A new window called "New Referral - Grandview Kids - Client Services" will appear, where you will add the patient's information. Please note that before you can begin your e-Referral, you will be prompted to sign in with your Ocean account.

All fields with an asterisk (\*) beside their title require completion.

Once you have added patient information, you will be asked if the patient or legal guardian is aware of and agrees with this referral. Click "Yes" or "No." You can add relevant attachments by clicking the grey "Add Attachments..." button.

After selecting "Yes" to the above agreement, you will input additional patient information.

Your information will auto-populate under the "Referrer's Information" section. Confirm that everything looks correct, then click the "I accept the Terms of Use" field.

The "Ocean Referral Directory Regional Authority License Agreement" will now appear. Once you have read and agreed to the agreement, click "I Agree."

Click "Send Referral."

Confirm you have obtained consent from your patient to share their email. Click "Consent given - use email."

Click "Correct - Proceed"

You will receive confirmation that your e-Referral was sent successfully. You may now print it by clicking the blue "Print" icon. To complete your e-Referral, click "Close."
