Alert! - These instructions are for a computer only Requirements: Computer or Phone to complete the form Please have all immunzation records and Photo ID downloaded to your computer or in your photos app on your phone prior to following the instructions below

Double-click on Google Chrome, Safari, Firefox or Microsoft Edge on your computer

In the address bar type www.cnos.net

Click Forms in the upper right of the CNOS website

Scroll down and Click Immigration Physical Form

Under Step ONE: Click Immigration Medical Exam (Form I-693)
Alert! Fill out Part 1 and Part 2 Only of this Form

Fill out this form Part 1 and Part 2 ONLY

Once you have Filled out Parts 1 & 2 then Click on the download icon in the upper right

Click "with your changes"

Click Desktop under This PC

Make sure the File is named i-693

Click Save

Close this tab and go back to the cnos website tab

Under Step 2: Click Choose File

Click Desktop on the Left

Select the i-693 file we saved in the previous step

Click Open

Under Step 3: Upload your Photo ID

if you saved a digital copy of your photo ID to your desktop

Select the File you saved of your Photo ID and click Open

Under Step 4: Immunization Records then Click Select files

Select the File you saved of your Immunization Records

Click Open

Under Step 5: Fill out the required Information

Check an Appointment Preference

Check I Agree

Click "I'm not a robot"

Click Submit
