All requests must be accompanied by the corresponding legal documentation showing the change and a **signed** Social Security card matching the new name. - Legal documentation can include a birth certificate, marriage certificate, government-issued identification card, driver's license, passport, or court order. - For non-U.S citizens, an updated passport can be used in place of a signed Social Security card.
