Click "Access Leon’s Law Forms"

Click "Acknowledgement Form"

Fill out all the required information on the form \*Please include your 7 digit South Piedmont user ID#. You can locate your South Piedmont ID# on the acceptance letter you received from our admissions team when you first applied to the college.

Please fill out information for "First Parent / Guardian" and "Second Parent/Guardian" (as applicable)

Click "Submit" when the form is completed
