MDI Meal Estimation with Sliding Scale Correction - Start a Patient

This guide walks healthcare providers through setting up a Multiple Daily Injection (MDI) plan for a patient who estimates meals with a sliding scale. 01/01/2026

Next, set the patient's treatment plan settings: 1. Click the toggle for "MDI - Meal estimation with sliding scale correction." It will turn blue. 2. Click the toggle for "Type 1." It will turn blue. 3. Click the toggle to choose the patient's pre meal glucose target range. It will turn blue. 4. Click the blue "Start Patient" button. Click here

Review and confirm the patient's settings. Click the blue "Confirm" button.

You can now enter the patient's treatment plan. This is done only for the first time starting a patient in the endo.digital system. First, enter the patient's Basal Plan. 1. Enter the time of insulin delivery: - Click the hour drop down to choose the hour, - The minute dropdown to choose the minutes in intervals of 15 minutes - Adjust the time of day, AM or PM. 2. Enter the basal amount by typing in the number of units in the text box. 3. Select the insulin type from the dropdown menu. 4. If there is more than one delivery time, click "Add a new row". 5. Click the blue word "Done" to complete editing the recommended Basal Plan. 6. Click the blue "Next" button to adjust the Bolus plan next. ***You can always scroll down to the bottom of the screen to view the patient's data reports: AGP, Daily, and the Logbook.***

Now enter the patient's recommended Bolus Plan. First, complete the Meal section. 1. Select the insulin type from the dropdown menu. 2. Then enter the insulin amount, in units, for size of meal. - If the amounts are the same for the whole day, type the numbers into Breakfast and click the blue "Copy for entire day" button. - Click the blue button "Yes" to proceed. 3. Click the blue word "Done" to complete the editing of the Meal section.

Second, complete the Bolus Correction section. 1. Select the toggle for correction type: No correction or Sliding scale correction. 2. For entering the sliding scale correction table, you have 2 options to choose from: 1. Use the 'Adjust table' functions: - Enter the glucose value the patient should start his glucose correction from, in the "Start correcting from" box. - Enter the correction factor in mg/dL/U (that will indicate the glucose 'jumps' in the correction table) in the "Correction factor" box. 2. Enter the table parameters manually: - Enter the maximum glucose value for each insulin unit increment in the "To" column. - Enter the amount of units for correction per each range of glucose values in the "Insulin Amount", per time of day. - If the amounts are the same for the whole day, type the amounts into the morning column and click the blue "Copy" button to adjust the values across the entire day. 3. Click the blue word "Done" to complete the Correction section of the bolus plan. 4. Click the blue "Next" button.

The final step is to approve the patient's recommendation. 1. You may enter additional instructions or message to the patient by typing into the "Message to Patient" text box. 2. Review the basal plan 3. Review the bolus plan. 4. If you are satisfied with the plan and are ready to record it in the patient's medical record, as well as deliver to the patient in the DreaMed Diary App, click the blue "Approve" button.

To return to the patient's dashboard, click the patient's name at the top of the screen.

Done! The MDI - Meal estimation patient has been started successfully! Now all of the patient's glucose and insulin metrics are auto-populated in the EMR flowsheets and the treatment plan is captured in the EMR encounter Notes section. You can also see the date the recommendation was approved and that it was accepted in the Diary app.
