Patient Education - ePRO Self-Care Plans

Patients submitting ePRO symptoms receive a real-time response to their symptoms via a Symptom Plan with education and self-care instructions

Tip! Patients answer questions to report any symptoms they are experiencing

Click "GET STARTED" after navigation to the assigned question set from the app, or link

Answer all questions including symptoms and any attributes

Click "SUBMIT"

NEW! Symptom Plan via Symptom Graphs

After a patient answers their ePRO health question, the Symptoms card on their PROmpt Home page will indicate that new information is available

The list view may be expanded as desired

The patient's symptom experience is visible them both in graph and in the text displayed at the bottom of the symptom card

When an organization's settings support automatic symptom education plans, a new Symptom Plan button will appear in the card

With this selection, they are automatically navigated within their Symptom Plan to education about the symptom they have reported

Click "Chills" (for example)

Click "NEXT" to move through the education pieces

Click here to navigate to higher categories of information within the plan

Plan Search & NCI Dictionary

Tip! Within a Symptom Plan, patients can search the document and access the NCI Dictionary of cancer terms

Click "plus" button on lower middle of screen

Select the book icon to navigate to the NCI dictionary of cancer terms

OR Select the magnifying glass to enter search terms for the plan

After selecting the book icon to access the NCI Cancer Terms Dictionary, Click the "Search" field

Enter desired term (example: "fever")

Click "Search"

Click term (example: "fever")

Click "CLOSE" when finished

Symptom Plans via Home Page

From the Carevive PROmpt® Home page

Select the Documents card

Select "Symptom Self-Management Plan" to view content

Tip! The available plan addresses symptoms that were received in the most recent patient response

Click "Your Symptoms" category to view any available topics

Click "Chills" to see information and self-care for the symptoms reported

Tip! Patient education is delivered in 'recommendation' style and is written for an 8th grade reading level max. Each recommendation is supported by evidence and includes 'tasks' to support a patient's understanding and self-care participation
