Gather your banking information and navigate to Metrix Enroll: [coupk.metrixenroll.com](http://coupk.metrixenroll.com)

After logging in, click on "Payment Setup Form" to navigate to the form

**You cannot save your progress in the ACH Authorization Form.** If you navigate to another page in the same window or log out, the information will be erased. This is to protect your banking information.

**Enter your banking information** exactly as shown in your account, on a check, or from an official bank document.

"Name of Account" should be the name your bank would associate with the account. This may be different than an authorized signer or the individual completing the form.

You will need to **confirm your banking information** by entering your Routing Number and Account Number again. The numbers must match to be able to submit your form - the system will let you know if something is off.

You can find your routing and account numbers on a check, or often times, through online banking. Routing numbers are always 9 digits, but account numbers will vary.

You have the **option to upload** a voided check or official bank letter with your account information. **This is not required**, but can help with the account verification process.

You must **check the box for authorization acknowledgement** **authorization acknowledgement must be checked.** This constitutes the same authority as a signature.

**Once submitted, the form entry will be erased** and the payments team will proceed with the verification process. Take a moment to double-check the information provided!

Once all required fields have been completed, the **Submit** button will become available at the bottom right of the form.

After submitting, the system will reload to your payment reports dashboard page. There will be a brief confirmation in the bottom right corner, but you will not receive an email confirmation. We will let you know if we need any additional information.
