Physician Script: Physician Printed Name and NPI automatically filled in

This is an optional feature. Level 4 users can enter the physician’s name and NPI number, which will automatically populate the physician script forms. This is the workflow they will use.

Click "Adjust My Settings"

Click "View" in the Coop Physicians section. This will be updated to say District/Coop Physicians.

Enter the physician information. Click "Save"

Click the "Close" icon to exit the Coop Physicians settings

Click "View Form" for either the Physician Script - without dates or for the Physician Script - with dates.

Physician information entered in Adjust My Settings will automatically populate on the Physician’s Script for students.
