From the home page (<https://outreach.ctipractices.com/>), select the “Visit Form Filler” button.

This will lead you to the form filler page. Under the "Recent Form Drafts" header, you can see any assessment that may be in progress which will be listed as in "Draft." To see a full list of all drafts and submitted assessments, select the "Show All" toggle on the righthand side of the page. This is also where you can start and complete any assessments.

To start a new assessment, use the Start New Forms and Assessments button on this page OR the clipboard icon on the bottom navigation to see any forms and assessments that you can complete.

From this page, select the form you wish to start and that form will open in a new tab. The home button on the left of the bottom navigation bar will lead you back to your recent form submissions.

Select the right button on the bottom navigation bar to lead you back to your Outreach dashboard.
