Registering as a Portal User with the Oklahoma State Department of Health: Consumer Health Service

Follow this guide to successfully create and set up your account on the Oklahoma State Department of Health: Consumer Health portal. This walkthrough covers the email verification process, security setup, and profile configuration required to access your account.

Navigate to either the **Individual Licensing Application Portal** or the **Establishment Licensing Portal** to create your **portal user account**.\ **Note:** Once you create login credentials, they can be used across the Oklahoma State Department of Health Consumer Health Service portals. **Application Portal:** <https://odohcs.portalus.thentiacloud.net/webs/portal/application/#/login> **Establishment Licensing Portal**: <https://odohcs.portalus.thentiacloud.net/webs/portal/business/#/login>

Example: Click "**Application Portal**"

Click "**Register for an Account**"

Enter the **email address** you want to use to sign in.\ After entering your email address, click "**Next**."

Access the inbox for your entered email address and open the Thentia Cloud email: **You have Created An Account**.

Copy the **Temporary Password** provided in the account creation email.

Paste or enter the **Temporary Password** and then click the "**Next**."

Enter your **Personal Information**.\ Including your first name, last name, and birthdate. (You must be at least 18)\ Then click the "**Next**" button.

Create your **Password**.\ Be sure to follow the password stipulations displayed on screen.\ Then click "**Next.**"

Choose one authentication method out of these two options, for logging in in the future:\ **Option 1** - Use **email security code** sent to registered email (Recommended)\ Option 2 - Answer security questions (Shown on later step of document) Click "**Next**"

Access your **Inbox.**\ Open the "**Security Code**" email.

Copy the provided **Security Code.**

Switch back to the tab Oklahoma State Department of Health Consumer Health Service. \ Paste or enter the **Security Code.**\ Then click the **Submit** button.

You now will be able to access the Application Portal with your email and password. **Remember**, you will need to access the Temporary Password that will be emailed to you after you enter your email address and password.\ \ **Note:** Steps to set up your access with the **Security Questions** option explained below.

Portal User setup: **Answer Security Questions** option. **Two-Stage Authentication Step**\ After a portal user has: \ -Registered for an account by providing an email address\ -Retrieved and entered the **Temporary Password**\ -Entered their First Name, Last Name, and Birthdate Two-Stage Authentication Step: Select the **Answer Security Questions option** and click the **Next** button.

For each of the **Security Questions**, click on the dropdown and select a question for each. (You cannot use a question more than once)

Select your questions for each of the three **Security Questions**.

Enter your answers for each question. **Keep in mind** that the answer fields are **case sensitive**. **Note**: You will only have three tries to correctly enter the answer for the security question; if you are unsuccessful after one or two tries, it is recommended to return the initial login screen for the portal and click on the Forgot Password? Reset Password option. You will then have an opportunity to not only reset your password but also select and set up your Two-Stage Authentication Method. (Security Code Email is recommended) Then, click the **Next** button.

You now will be able to access the Application Portal with your email and password. **Remember**, You will only have three tries to correctly enter the answer for the security question; if you are unsuccessful after one or two tries, it is recommended to return the initial login screen for the portal and click on the **Forgot Password? Reset Password** option.
