- **If you have received a message that you already have an account OR do not know your password:** Click 'forgot password' on the agreement portal log in page - **If you do no not know your username, cannot recover your username (*all user names are first name.last name)*, or have trouble logging in, please do not hesitate to email us at**[MTA@childrens.harvard.edu](mailto:MTA@childrens.harvard.edu)

**Navigate to** <https://bch.inteum.com/bch/agreementportal/login.aspx>

**Click on request account**

**Enter BCH email address and type code requested**

**A pop up will appear notifying you of your request, click ok**

**You will receive an email with a link to complete the following** - Your user name should be your name within your BCH email address. - Example[jane.doe@childrens.harvard.edu](mailto:jane.doe@childrens.harvard.edu) **your user name would be Jane.Doe** OR if you have a number in your email as [jane.doe1@childrens.harvard.edu](mailto:jane.doe@childrens.harvard.edu), **your user name would be Jane.Doe1** - Add as password - Fill in required information - Providing additional requested information will be beneficial for continued work with BCH

**Upon completion, click on "create an account"** - Log in with account information - Once logged in follow prompts to create an MTA request
