The title should be short, informative, and non unique.

Select the type of device

Usually these will be desk phones

Select the requested action to display necessary programming information.

Move and Modify will be the 2 most common

Add the Users HWS assigned extension here *NECESSARY INFO*

Add location data, building floor and room/port This helps to keep phone location information up to date, and find particular phones in instances where a user has multiple phones or is used departmentally ex:4357 NOT NECESSARY, HIGHLY USEFUL

Primary Extension (Current) needs to be filled in as the number currently assigned to the phone being requested to be changed.

Within the description, add any relevant ticket information ex: if a user emailed the request, copy and paste the content of the email request in the description. Using a format to obtain how they want their phone to look and act helps as well, some users want numbers on their devices to be there but not ring. 0 (Primary extension): 1:SCR/SCN-#### (Ringing/Non-Ringing) 2: 3: 4: 5:

Select the "Affects Individual (or is informational)" option.

Select the "Work slightly impacted" option.
