Departments looking for a certificate of insurance showing evidence of the University of Maryland's Property or Liability insurance coverage must provide Risk Management with a copy of the agreement between the University and the service provider requesting proof. At minimum, the copy should include the following information:
(1) The provider with which the University is contracting
(2) The service that is to be provided
(3) The signature page
(4) The relevant section that speaks to the request
