SIPA: How to Submit a Host Organization Questionnaire

A step-by-step walkthrough for Stanford students to request a Host Organization Questionnaire. You can submit this request to your host org prior to submitting your SIPA applicaiton.

Go to the "Host Organization Questionnaire(s)" pane on the right-side of the application form.

Click "I understand"

Provide the following for your host organization contact who will complete the questionnaire: name, email, and title (e.g. Executive Director, Program Assistant, etc.)

Click "SIPA Host Organization Questionnaire"

Complete the "Waive Rights" section

Click "Submit." Your host organization contact will receive an email from SOLO with a unique link to complete the questionnaire.
