Submitting a Certificate Request for SB County 4th District Supervisor Curt Hagman (External)

Navigate to <https://bosd4.sbcounty.gov/>

Click "MENU"

Click here. (Online Forms)

Click "Certificate of Recognition"

Click the "Enter Your Org Name" field.

Type the name of the contacts information

Click the Date of Occasion Box to Choose the Date of the Occasion.

Choose your date.

Enter the name of the Occasion/Event

Enter the Name of Your First/Only Honoree

Add a Title/Position if you would like. This is an optional field.

Enter what you would like to see on the certificate. It is important to type exactly what you want as this is how it will appear on the certificate.

If you have more then one honoree click the Add Another Honoree Button. (If not Skip to Step 15

You will repeat steps 10-13 for each additional honoree. (You can submit 50 honorees per online form.)
If the honorees are all being honored for the same thing and each certificate can read exactly the same, please copy and paste the recognized for section from your previous honoree and simply keep pasting in the recognized for section for subsequent honorees.

When you are finished entering 50 or all of your honorees, click the finished button. This will submit the form and upload the certificates into our system. (Read the remainder of this how-to for important information)

It is Important that you wait to navigate away from this website until you receive this confirmation page. If you do not wait for this confirmation, then the upload has not been completed, and we may not receive your submission.

Check your Email inbox for an email from Office of Supervisor Curt Hagman. This may take a couple of minutes. The Subject will be Thank you for submitting a Certificate of Recognition request. (If you do not receive an email, your request may have not gone through. Please Call 909-465-5265 to verify)

Use this email to verify that the information entered is correct and that all of the entered honorees are included in the submission.

Please also verify your contact information is correct.

This section will show your honorees.
Alert! If you have more than 50 honorees, please call Ruby Long at 909-465-5265, as there is a different process for larger requests.
