Navigate to **"Claims"** in the top menu bar. Click **"Submit a Claim"** from the drop down menu.

The practitioner's details will appear in the top section. If the office has multiple providers, **please take a moment to review the information carefully** and ensure that claims are submitted under the correct practitioner. Scroll down to the **Patient Information** section and complete the required fields. Ensure all details are entered accurately, including name spelling, birth year, and identification number. ***Important:*** - *Incorrect entries will result in errors and processing delays.* - *Take note of the blue icons (tooltips) throughout the portal. These are designed to provide guidance and ensure a seamless process as you navigate the portal.*

Click **"NEXT STEP"**

**HEALTH PRACTITIONERS ONLY:** The patient **"Coverage Summary"** dropdown will appear. Click the arrow to view more information including: - **Coverage** - **Benefit Maximum** - **Benefit Period** - **Amount Remaining** - **Deductible** Plus, additional details pertaining to the patient and coverage. ***Important:*** - *This feature is unavailable for **Vision Care Providers** at this time*

If the claim is related to a **motor vehicle accident** or **workplace injury**, select **"Yes"**. The portal will prevent further submission and prompt you to submit the claim to the appropriate agency as the first payor. If there is any remaining eligible amount, the member may then submit their claim to our office for review. If **"No"** is selected, the portal will allow you to proceed with the claim submission.

Input the required fields: - **Description:** Select the **service type** and **duration** from the dropdown menu - **Date:** Select the **Date of Service** from the calendar - **Amount:** Input the total claim amount *Providers have up to 90 days to submit a claim.*

Click **"ADD ANOTHER SERVICE"** to add another service or claim the **same** member incurred. You may add up to 10 claims within a single submission. Click **"NEXT STEP"** to proceed.

Before clicking **"SUBMIT"**, carefully review all claim submission details to ensure accuracy. Double-check patient information, claim details, and provider selection to prevent errors or delays in processing.

Click **"PRINT CLAIM"** to generate an **Explanation of Benefits (EOB)**. You may print the EOB and provider to the member or, save to your device and email to your patient.
