Submitting a Food Exception Form

This guide provides a step-by-step process for submitting a Food Exception Form at Salisbury University, ensuring compliance with university policies for food at events. This process replaces the previous Microsoft form. Students are able to submit an Exception to the SU Food Policy Jira Request. These requests should be submitted 2 weeks prior to the event date to allow sufficient time for approval.

Click "Salisbury University"

Click the "Email address" field.

Type your SU student email address

Click "Next"

Click "Continue with Atlassian account"

Once logged in, Click "Gull Card, Event and Door Access Support Center"

Click "Event Access"

Click "Apply for an exemption to the SU Food Policy"

Click 'Raise this request on behalf of". Note: If the **student is submitting the request themselves**, their name will auto-populate in this field. If a **staff member is submitting the request on behalf of someone else** (a student, staff member, etc.), they should enter that individual’s name so the requester can follow the progress of the submission.

Click the "Summary " Field. Provide a brief description of the request. EX: Student Org Food Exception Request

Click the "Description" Field. Provide a brief description of the event. EX: The Dinosaur Club is having an event open to campus and we would like to provide outside food for attendees.

Click the 'Date" Field. This should be the date of the event.

Click the "Event Location" field. Type the Name of the Location in which the event is occurring.

Click the "Menu Items Needed" Field. Type the food/beverage items being purchased. Ex: Soda, Chips, Candy, etc.

Click the "Suggested Vendor" field. This should be where the items will be purchased from. EX; Walmart, Food Lion, Sam's Club.

Click the "Expected Event Attendance" field. This should be a rough estimate for how many people you expect to attend the event.

Click the "Student Org Advisor" Field. The organizations on-campus advisor should be selected. If the student does not know their on campus advisor, direct them to either contact their CSIL POC or to check their Involved Roster.

Click the "Contact Number" field. This should be the requesters phone number, in case of any questions.
