Tech Hub 2.0: Completing a Form 1; Returning 3C2A Student-Athlete (Updated 9/19/25)

Alert! These below instructions are set-up for student-athletes using a laptop or computer to complete the below steps. If a student-athlete is using a cellphone their experience might differ slightly.

Navigate to [https://3c2a.my.site.com/form1/s/](https://3c2a.my.site.com/form1/s/)

Tip! Begin typing the name of your current college in the text box. A drop-down menu will appear as you type. Once you see your current college name appear, select that option in step 2.

Click on your current college's name in the drop down menu.

Click "Next"

Click here to confirm your college. If you have selected the wrong college, use the "Go Back" button on the bottom right side of the screen to resubmit the correct current college.

Click "Yes"

This guide is for student-athletes returning to the 3C2A **who have previously completed an online Form 1**. Select the "Yes" option.

Provide the last name provided on the prior online Form 1, date of birth and email address. **If anyone of these fields is different from what you provided on the prior online Form 1 the system cannot match your profile.**

Click "Next"

Alert! The college administrators can look up your prior information to ensure you are matched correctly in the eligibility system. **Do not create a new profile!**

When you are successfully located the below message will populate. The next step is to establish your user account information to access the returner Form 1 online.

Click "Next"

**Establishing a Student Account** Go Check Your Email to establish your profile. **Username will be your email address.**

Example below of the email you will receive as a **returning 3C2A student-athlete**. The email comes for School Portal - [techhub@3c2asports.org](mailto:techhub@3c2asports.org) The **username** will always be the **email address** provided on the online Form 1.

Click "LINK" to establish a password.

**Establish a password that you should keep track of to login as needed**. Click "Change Password"

It will redirect to the 3C2A Tech Hub Portal homepage.

Click either the tile/menu bar "**Submit Your Form 1**" as a returning student-athlete to complete your Form 1.

Tip! Begin typing the name of your current college in the text box. A drop-down menu will appear as you type. Once you see your current college name appear, select that option in step 2.

Click on your current college's name in the drop down menu.

Click "Next"

Click here to confirm your college. If you have selected the wrong college, use the "Go Back" button on the bottom right side of the screen to resubmit the correct current college.

Click "Yes"

**[[20]]** Enter the **Month** and **Year** of when you started classes at your **current college**.

Select your current sport this season.

Confirm your current sport again for accuracy.

Select the best response to a question related to your **amateurism** status.

Click "Next"

Please carefully review your **contact information** and provide any updates. You can update your email address, phone number, Student ID number, Race/Ethnicity (same options as CCCApply application), current housing information (if unhoused select that checkbox). High school information for your review.

The section named "My Last Form 1 Responses" displays your prior responses to questions regarding your amateurism status, the Felony Disclosure Form (if your college opted into this digital form) and if you are allowing the use of your Name, Imagine and Likeness (NIL). **If you would like to make changes, those opportunities will come in the next steps within this online form process.**

After you have reviewed your profile information and did the following: - **Made Changes**, click "Update" *or* - If you have **no changes** to confirm your information, please click "Update"

Alert! The **Sports Participation** table **will not** automatically populate your current sport for 2025-26. Do not enter it in as it will populate after you complete the rest of the information and submit.

If you did **NOT** participate (e.g., not on the Form 3 Roster) you will see nothing in the table. Click "Next"

Tip! If your prior participation history **is not listed** **prior to 2025-26** please add that information.

Review your Sports Participation for accuracy. If it's correct, click "Next" if you need to add information go to step 27.

Click "Add Additional Sport" if you need to add participation.

Click "Sport"

Select a Sport Sponsored by the 3C2A.

Add the name of the **College**.

Please add your participation level in the sport. **Varsity** - You competed in a game/meet/match.\ **Practice Only** - You did NOT compete, but practiced (e.g., redshirt/grayshirt).\ **Athletic Waiver** - You received a waiver (e.g., medical hardship, COVID, Extenuating Circumstance, etc.)\ **Club** - Not a team sponsored by the athletic department.\ **JV** - Junior Varsity practice and competition.

Select the "Academic Year" you participated.

Click "Create" to add that sport participation.

Please review to ensure all your Sports Participation is accurate. Either Add **Additional Sport** or click "Next" to advance to the next section of the form.

Please review your **Life Experience** and add any new experiences not listed or that is missing. This includes the following Life Experiences **after high schoo**l: College attendance (e.g. even online), Employment, Unemployment periods, Military Service, Church Mission and Other/Unknown (e.g., gap year, travel). If you have nothing to add click "Next" and go to step 46, Felony Disclosure Form (if applicable).

If you need to **add a Life Experience**, click "Add New Experience"

You will add the Month and Year, Experience Type, if still present click that box or if completed select the end Month and Year.

Add the **name** of the College, Employer or type of Life Experience. Ask a college staff member if you have questions on what to share in this section of the online Form 1. Add the **City** and **State** of the Life Experience.

Review all the fields for accuracy before you click "Create"

Click "Next" if everything in the table is accurate and complete. **If you need to make an edit to the information follow the below steps**. If not go to step 45.

To **Edit** any of the fields, click on that row and cell. A pencil icon will appear and you will either be able to type in information or select from a dropdown menu. Click outside of the cell afterwards and a "Save" button will appear at the bottom of the table.

To update the State or Other-International Address select from the dropdown menu.

Example below of what you will see once you make an edit.

Click "Save" to save the edits or updates.

Click "Next" to advance to the next section of the Form 1.

Alert! Your institution might require you to complete a paper **Felony Disclosure Form**. If so you will not see this option to complete an online Felony Disclosure Form.

California Educational Code requires compliance with a Felony Disclosure Form, please mark the declaration that applies (Choose one).

Click "Next"

Please carefully review this brief summary of **3C2A Eligibility rules**. If you have any questions please contact your Athletic Director or Eligibility Specialist. Click "Next"

By checking the next set of required checkboxes you are acknowledging the following, "*I certify that the information I have given is true and that I have read and understand the requirements to be a student-athlete at a California community college*."

By selecting this checkboxes you are acknowledging the following, "*I agree to disclose my education records and authorize my institution to request information from my previous college(s) to help determine my 3C2A eligibility. I understand that these records will only be available to authorized representatives of this institution, the athletic conference and the 3C2A*."

By selecting this checkboxes you are acknowledging the following, "*I understand that there are special rules for student-athletes. I understand and will abide by the above statements and all rules of athletic eligibility, information falsely given or concealed by me will cause my college's forfeiture of games, meets, and matches in which I competed, and that I may be designated ineligible for future competition*."

The last checkbox is **optional**. By selecting this checkbox you are authorizing the use and publication of your Name, Image, and Likeness (NIL) and athletic records, by the 3C2A and it's member institutions.

Click "Next"

Click "Finish" to successfully submit your Form 1.

Bookmark this website. You may receive an email asking you to make updates or changes to your information by the college. You can logout by clicking on your name in the fare right corner.
