Tech Hub 2.0: Creating a New Form 1; Transfer Student

This guide is essential for first-time transfer student-athletes from outside of the 3C2A navigating the Form 1 eligibility process at California community colleges. Tip! Begin typing the name of your current college in the text box. A drop-down menu will appear as you type. Once you see your current college name appear, select that option in step 2.

Click on your current college's name in the drop down menu.

Click "Next"

Click here to confirm your college. If you have selected the wrong college, use the "Go Back" button on the bottom right side of the screen to resubmit the correct current college.

Click "Yes"

Alert! The next steps are for student-athletes who are new to the 3C2A. Transferring from another College or University outside of our association.

This guide is for First Time Transfer Students. Select the "No" option.

Click "Next"

Enter your **first and last name**, preferred name, and **phone number**.

Tip! Use a personal email address verses a college email address. This ensures the student-athlete can access their profile if loss access to their college email account.

Enter your **personal email** address.

Enter your current college **student ID number**.

Select the **gender** option that applies to you. Options included Female, Male, and Non-Binary.

Select the **Race/Ethnicity** that applies to you. The options below are the same as the options you selected on your CCCApply application.

Enter your **date of birth**.

You will have the option to select "Unhoused" or type in your address information.

Use these fields to type in your current address.

Enter the information for the last high school you attended.

Enter the information related to the last date attended/graduated using the month and year.

Click "Next"

Enter the **Month** and **Year** of when you started classes at your **current college**.

**Transfer Student-Athletes** will add their prior collegiate enrollment within this section of the Form 1. It allows for up to seven (7) colleges to be reported. If the student exceeds this number of colleges attended they can provide a written statement of their enrollment history for the college to keep on file and use for eligibility certification.

Select the "Yes" option, if the student-athlete has **attended another college besides this one after high school.**

Provide the name of the **college** attended.

Provide the **city and state** where the college is located.

Provide the **Month** and **Year** enrollment started at the college.

Click this dropdown to indicate if the student-athlete is still **attending** the college.

If the student selects "No" they are not attending, the Form 1 will request they provide the last date of attendance by **Month** and **Year**. If the student selects "Yes" they are still attending, proceed to the next step.

The Form 1 will ask if the student-athlete has attended another college. The student-athlete can provide up to seven (7) colleges they have attended.

Click "Next"

Alert! The **Away from School** section of the Form 1 provides important information for the eligibility certification process. This allows the student to disclose anytime periods away from college. A student-athlete can select any or all of the options.

If you have taken a gap year or other extended time away from school, select yes, and enter the information required.

Click in the check boxes that best describe the time you spent away from college.

Tip! Each area allows for five (5) types to be shared and anything beyond that can be provided by the student as a written statement and kept on file with the college to assist with eligibility certification.

Please, input the information in chronological order. Select each box that applies to your extended time away from school. Once complete, please click "Next."

Below is an example of an additional section to be completed by the student, if they have another item to share (e.g., another employment period/different job).

Click "Next"

**Athletic Participation History** section. Please provide the current sport you will be participating in this season. This section of the Form 1 you will provide your participation history for this sport and any other 3C2A sponsored sports you have previously participated in (e.g., cross country / track & field).

Select your current sport this season.

Confirm your current sport again for accuracy.

Have you previously participated in this sport at the collegiate level? As a transfer student-athlete the answer should be "Yes." Level of participation includes: Varsity, Practice Only, Athletic Waiver (e.g., Medical Hardship, Extenuating Circumstance, etc.), Club and JV.

Provide the name of the college, academic year of attendance, and level of participation.

Select the "Yes" option if there are additional years of participation.

You have up to five (5) seasons of your current sport you can share participation history for in this section of the Form 1. Select the option of "No" once all your participation history in the current sport has been shared.

Select the best response to a question related to your **amateurism** status.

Have you participated in **ANOTHER** sport in College? If "Yes" you will provide that participation history information here. For example, your current sport is water polo and you also participated in swimming and diving.

Click "Next"

Alert! Your institution might require you to complete a paper **Felony Disclosure Form**. If so you will not see this option to complete an online Felony Disclosure Form.

California Educational Code requires compliance with a Felony Disclosure Form, please mark the declaration that applies (Choose one).

Click "Next"

Please carefully review this brief summary of **3C2A Eligibility rules**. If you have any questions please contact your Athletic Director or Eligibility Specialist. Click "Next"

By checking the next set of required checkboxes you are acknowledging the following, "*I certify that the information I have given is true and that I have read and understand the requirements to be a student-athlete at a California community college*."

By selecting this checkboxes you are acknowledging the following, "*I agree to disclose my education records and authorize my institution to request information from my previous college(s) to help determine my 3C2A eligibility. I understand that these records will only be available to authorized representatives of this institution, the athletic conference and the 3C2A*."

By selecting this checkboxes you are acknowledging the following, "*I understand that there are special rules for student-athletes. I understand and will abide by the above statements and all rules of athletic eligibility, information falsely given or concealed by me will cause my college's forfeiture of games, meets, and matches in which I competed, and that I may be designated ineligible for future competition*."

The last checkbox is **optional**. By selecting this checkbox you are authorizing the use and publication of your Name, Image, and Likeness (NIL) and athletic records, by the 3C2A and it's member institutions.

Click "Next"

The final page is a summary of all the information provided by the student-athlete. **A college staff member or coach should review this with the student-athlete before they submit their information**. If any changes or corrections need to be made the student can click the "Previous" button at the bottom of the page to return back to that section of the Form.

Click "Previous" to go back and make any edits or add missing information.

Click "Submit" ONLY after all the information has been reviewed for accuracy and completeness.

Thank you for completing your online Form 1 eligibility form. **You will now need to check your personal email to establish a 3C2A Tech Hub portal account**.

Student Account Set-Up Process

Below is an example of the email each student-athlete will receive after completing an online Form 1. The email comes for School Portal - [techhub@3c2asports.org](mailto:techhub@3c2asports.org) The **username** will always be the **email address** provided on the online Form 1.

Click "LINK" to establish a password.

Establish a password that you should keep track of to login as needed.

Click "Change Password"

It will redirect to the 3C2A Tech Hub Portal homepage.
