Tech Hub 2.0: Opting In/Out of adding the Felony Disclosure to the Form 1 workflow

Navigate to [https://3c2a.my.site.com/schoolportal/s/](https://3c2a.my.site.com/schoolportal/s/)

Click "Sports Offered" from the tile or header

Alert! The option to add the **Felony Disclosure** will be at the top of the **Sports Offered** page annually.

Click this dropdown, which is defaulted to "Yes". To OPT IN, select Yes, to OPT OUT, select No.

Once you have made your selection. Click "Submit"

Alert! Alert! You only complete this request once per academic year. Contact the 3C2A office if your intuition needs to change their OPT IN/OUT status.
