Tech Hub 2.0: Reviewing Student-Athlete Form 1 Responses

This guide provides a step-by-step process for reviewing student-athlete (SA) Form 1 responses, ensuring that you can efficiently access and evaluate important data. By following these instructions, you can easily navigate the system, review individual responses, and understand how returning SA's updates in life experiences and sports participation affect the athlete's profile directly.

Navigate to [https://3c2a.my.site.com/schoolportal/s/](https://3c2a.my.site.com/schoolportal/s/)

Click "Roster / Form 3" from the header or the tile.

Click the hyperlink for the sport of the student-athlete that needs to be reviewed.

Click "Certify Student"

Scroll to the bottom of the page to "Form 1 - Raw Data"

Click the hyperlink on the last Form 1 Response listed in the table or with the latest Created Date. This is the first full Form 1 Response on file for the student-athlete.

The Form 1 response for the student-athlete will populate for review.

Tip! To return to the Student-Athlete (SA) Profile page click the web browsers back arrow button.

Reviewing Returning SA Form 1 Response Form

Scroll to the bottom of the page to "Form 1 - Raw Data"

Click the hyperlink on the first Form 1 Response listed in the table or with the most recently Created Date. This is the first full Form 1 Response on file for the student-athlete.

The returning Form 1 response for the student-athlete will populate for review.

Alert! Updates for **Life Experiences** and **Sports Participation** will populate on the Student-Athletes Profile view within those sections of their profile. Their responses will not populate on this Form 1 response view.
