Tech Hub: AD Review & Approval of Form 4 Appeal - Injury/Illness Waiver

This guide is essential for Athletic Directors navigating the Form 4 Appeal process related to injury/illness waivers. Athletic Directors will receive an email alert for a Form 4 Appeal pending their review and approval. The AD will then need to login to their 3C2A Tech Hub Portal.

Navigate to [https://3c2a.my.site.com/schoolportal/s/](https://3c2a.my.site.com/schoolportal/s/)

Click "Pending Approvals"

Click "Form 4 Appeal" pending review and approval.

**The AD has full access to make any updates to the appeal request.** This includes making corrections or adding additional files. Below are the steps to either request more information or a correction to process an appeal, if the AD is not making the corrections.

Requesting More Information or Correction

The AD can edit the appeal by clicking on a pencil icon in either the "Details" or "Checklist" sections of the appeal.

If the AD wants the Eligibility Specialist (ES) to make an update to the appeal they will select the radio button "Request Review."

Click "Next"

The Appeal will provide a text box to allow the AD to share what information is missing or correction is required. Click "Next" after typing in that information.

The appeal status will update to "Additional Info Required."

The "Appeals" list view will show a status of "Additional Info Required"

Alert! An email will be sent also alerting the college additional information is required to process the appeal request.

Approving a Form 4 Request

Click "Pending Approvals"

Click "Form 4 Appeal" pending approval.

Click on the radio button for "Approval"

Click "Next"

**Alert! DO NOT REJECT** the appeal, this will deny the request and it cannot be resubmitted. If the appeal requires corrections please make those edits before approving the request to advance the appeal to the Commissioner.

Click "Approve"

The Appeal has been advanced to the Commissioner. To monitor the status of the request click "Appeals"

The appeal status will update to "Under Commissioner Review."

Commissioners can request additional information, approve or deny the appeal. The college will receive email notifications for all of those status changes. Student-Athletes will receive the email with the approval or denial decision. All Form 4 Appeal -Injury/Illness decisions are final by the Commissioner.
