Update Member Information and Change Host Agency Guide

This guide provides step-by-step instructions on how a manager can update member information and change host agency in the NJ EMS Task Force systems. It includes specific directions on navigating the websites, filling out various forms, and uploading attachments. If you need to make changes to your own member information that does not allow you access, or to switch host agencies, contact njemsmacc@njemstf.org for further guidance.

Have member complete a change of Host Agency form and get signature of new agency representative. To download that form, start at the NJEMSTF Website

Navigate to [https://www.njemstf.org/](https://www.njemstf.org/)

Click "MEMBERZONE"

Click "TEAM CHANGE OF AGENCY"

A PDF Form will open that be filled, downloaded and printed to receive signatures. That should be sent to [applications@njemstf.org](mailto:applications@njemstf.org) when completed.

Log into D4H Team Manager <https://njemstf.team-manager.us.d4h.com/>

Click "Planning" then "Members"

Press Ctrl + f to open a search box, and search for member by name. Or scroll down list to member

Click on Members Name to change

Click "Update Details"

Click "MEMBER INFO"

Click this text field.

Type "Date - Make comment here about the change, from / to and who completed it."

Click "OPERATIONAL INFO"

Update "Host Agency" name to new agency

Click "GROUPS" Verify if any changes are needed

Click "Finished" and you should return to the members profile.

Click "Upload Attachment" to attach the signed Change of Host Agency

Click the "Private" field.

Alert! You have finished Team Manager updates, but still need to update Operations Center.

Switch to "NJ EMS Task Force | D4H Operations Center" <https://operations-center.us.d4h.com/#/njemstf>

Only ADMINs will have access. Click on your Profile Logo in upper right corner

Click "Admin area"

Click "Collections" arrow.

Click "Personnel"

Click "Search" and search by name, or scroll through list and double click on members name

Double-click on members name

Either Drag file to box, or Click to upload from your desktop. Click "Upload File"

In members profile, change "Host Agency" field to new information.

Tip! Make sure there are no changes to emails and/or phone numbers that need to be made here

Scroll to bottom of page and Click "Save"

Alert! Remember to upload signed document to Team Manager

Alert! Send an email to the host agency representative and member that change has been completed

Tip! Notify any Module Manager and the Leaders/Planners of the change.
