Uploading Invoices for Clinician Portal

Visit the portal at: <https://asianmentalhealthcollective.my.site.com/clinicianportal/s/upload-invoices>

Input your name. This should match the name you provided on your Direct Deposit form.

Fill in your practice name, if applicable. If you do not have a separate practice name, you can just input your own name.

Provide an email for us to send a confirmation email to. If you are in a group practice, this should be an email address that *you* or your record-keeper have access to.

Select from the options in the "Payment/Recipient" dropdown.

Insert your client's LTF code. Please triple-check to make sure this is correct. We use Therapy Fund codes to map onto the client— even a single missed digit could result in the incorrect client having a session logged in our system.

Select the Cohort that your session happened in. If you are seeing a client for an extension of sessions that were allotted for "**Q1 2077**" but it is currently beyond the original cohort's timeframe, **you should still be putting in "Q1 2077"**. We do this to ensure that extended sessions still map onto the cohort/quarter they were assigned to. If your client's cohort is not on this list, email us at therapy@asianmhc.org.

Fill in the session date using the date picker.

Provide the **single digit session # out of 8 total allotted sessions per client**. It is both you and the client's responsibility to understand how many allotted sessions they have used, especially if your client was seeing a different provider before finding services with you. This invoice portal only allows you to submit **one session** per usage, as per our recommendation of invoicing within a week of a session. If you have multiple sessions in a single invoice, you must fill in the portal multiple times. To smoothen this journey, we will auto-fill your Provider information upon finishing this use of the portal. More on that in a few steps!

Click "Go to Upload Invoice Screen"

Click "Upload Files"

Ensure there is a green checkbox before clicking "Done".

Click "Complete"

If that's all you need to submit, feel free to close this tab and check your inbox to and find your invoice confirmation. If you need to submit another invoice, click "Finish"

Your Provider information is pre-filled. Just fill in your Session Details again for another session!

[[After you have invoiced for the first time, you will receive]] two documents via DocuSign: a W9 for tax purposes and an ACH Authorization Form for payment.
