Sign into your Skyward family access account.

Locate the student you want to submit medical information for, and select the **Athletic Physical** tile.

In the ***Athletic Physical Upload*** field, upload the student's physical. When finished, check the ***I Agree*** box under the Terms of Agreement, then click **Submit Form** at the top.
