Here's an overview of which lines and sections CommCorp will expect to be filled out on your W-9 form:

S Corporations, C Corporations, and Partnerships

Line 1: Legal Business Name

Line 2: Disregarded entity name - if your business has a "Doing Business As" (DBA) name, it goes on Line 2. If not, leave this line blank.

Line 3a: Check the "LLC" box

Line 3a: Fill in Tax Classification ("S" for S Corps, "C" for C Corps, and "Partnership" for Partnerships)

Lines 5-6: List your business address

- If your primary business address is **different** from your Massachusetts-based address listed on your application, please list your primary business address. - If your legal address exists **outside of Massachusetts**, you can use the out of state address, but our team may conduct follow-up to verify your Massachusetts-based addresss. - Note that a member of the program team may contact you to confirm your business address if there is a discrepancy between what is submitted in your application and what is listed on your Form W-9

Part I: List your Federal Employer Identification Number (FEIN)

Be sure that your FEIN is consistent across all of your application documents.

Part II: Signature & Date

Individuals, Sole Proprietors, and Single-Member LLCs

Line 1: Name of individual

Line 2: Business name

3a: Select "Individual/sole proprietor"

Lines 5-6: List your business address

Part I: Employer Identification Number (EIN)

Although the IRS requires Individuals & Sole Proprietors to fill out their Social Security Number (SSN) in this section, CommCorp does not collect SSNs. You may redact your SSN when you submit your W-9 with your application.

Part II: Signature & Date

Line 3a: Write "Nonprofit Corporation"
