ePRO Symptom Check-In for Pathways

When a care team member has used an ePRO symptom pathway and provided recommendations to a patient, the system automatically checks in with the patient after a couple of days to see if the concern has been mitigated

Patient Experience

These come to patients via usual methods, as tasks for completion (Home Screen access shown here)

Will display as "Symptom Check-In"

Click "GET STARTED"

Any symptoms associated with the pathway that was used will be available for the patient to indicate a new status (in the example here, the Mood Health pathway was used). If multiple pathways were used on multiple days, the system will collate the information to have the least number of check-ins possible

The ePRO attributes are presented as with the original report

Patients can indicate helpfulness

And add comments

Care Team Viewing

If the ePRO has been reported as elevated, the system will provide the notification to the care team

The system will also record the response in the ePRO graphs
