Scholarly Collaboration and Research

Click the "Entity Name:" field and report the entity.

Click the "Status Update" dropdown and pick the appropriate status: - **New** - Activity has never been disclosed before - **No Changes** - Activity has been disclosed before and there are no changes - **Updated** - Activity has been disclosed before and there are changes (e.g., the activity has ended)

Select the appropriate "Activity Status": - **Activity Ongoing -** Activity is currently underway - **Activity Pending** - Activity has not yet begun but is being disclosed in advance - **Activity Ended** - Activity has been completed. Please provide the date the activity ended.

Select the appropriate "Domestic Status" and if it is Foreign, provide the country.

Provide the appropriate answer for whether you "Were Paid by Organization".

Click the "Type of Organization" drop down and select the appropriate type.

In the field, "Describe the Nature of Your Activities with this Organization:," briefly describe the activity.

Select the appropriate "Estimated Hours Spent Annually" on the activity. If it is above 100 Hours, pleas provide an exact amount.

Click "+ Add Another" if you have any other Scholarly Collaboration and Research activities to report.
