Navigate to the  "Inspect" service module.

In the top navigation, click "Form Builder"

On this page, you can view all of your forms. In the top right-hand corner, click on the "Add Form" icon

. . . create a new template by typing in the name of the new form. (Note: You can edit this name later.)

Select "Create" to start creating your new form.

To add a new component, click the "Add Component" icon. Components house your inspection questions. Group similar questions (e.g., "Fluid Check") in a single component.

Add a title to the component. (For example, "Safety Equipment" or "Steering and Suspension")

Under "Check Points" add the question and/or specific area name that should be inspected. 

For example, within a component titled "Engine," a check point might be "Battery."

Use the "Type" selector to identify how the operator should respond. 

For example, the checkbox feature will allow the operator to choose pass or fail. A photo type will allow the operator to take a photo.

Choose if the check point should be required. 

If required, the operator cannot submit the inspection form without answering the required check point.

The "Checkbox" type offers additional choices per check point. Choose if the alert recipient (such as a mechanic) should be emailed upon a failure.

Additionally, the checkbox option allows you to create question dependencies. A dependency is an additional question that will be presented to the operator if the checkbox passes or fails.

Once you add a dependency, add the question and/or specific area name that should be inspected. Then, select the question "Type" (alphanumeric, photo, etc.).

Add additional dependencies to your check point as needed.

Add additional check points by selecting the add icon to the right of "Check Points."

Once you have added all check points and dependencies, choose an icon for the component.

Click "Add" to add the component to the form.

Repeat these steps to add all components.

Tip! "Checkbox" check point types allow operators to choose pass or fail. If you've chosen to "Alert on Failure," you will want to add alert recipients to receive those failure alerts.

Select the "Alert Recipients" navigation tab.

If you have chosen to alert upon checkbox failures, add an alert recipient by typing their name into the email field.

Select "Add" to add the email to your recipients list.

Tip! Repeat this step to add multiple alert recipients.

Select "Schedule" to save your alert recipients.

Tip! Upon asset assignment, the form will immediately become available for operators to use. If you are still drafting the form, we suggest you assign assets last.

Select the "Assign Assets" button on the far right-hand side of the screen.

In the "Assign template Assets" window, the "Filters" allow you to filter the available and assigned asset lists.

The "Available Assets" list the assets that do not have access to the form.

The "Assigned Assets" list the assets that have access to the form. In other words, if an operator selects an asset in this list for an inspection, the operator would be able to view and select this inspection form.

If desired, start by filtering assets by branch. This can be helpful if you're creating a branch-specific form, or you'd like to alert different individuals per branch regarding reported issues.

For instance, you might have tags categorized by asset type. This filter is useful if you want to narrow down and choose assets based on a specific filter type, such as "Street Sweepers."

Click "Select All" to select all filtered assets. Or, select individual assets from the list by clicking on each.

Once selected, click on the arrow to move the selected "Available Assets" to the "Assigned Assets" list.

If you need to remove an asset from the "Assigned Assets" list, no problem. First, select the asset.

Now, select the left-pointing arrow to move the "Assigned Asset" back to the "Available Assets" list.

When finished assigning assets, click the "Assign" button. Now, when an operator selects one of these assets, this form will be available.

Once you have clicked "Assign," you will see all assigned assets listed.

If desired, flip the toggle to enable a "Compliance Template."

Compliance Templates offer three unique benefits. 1) This enables users the ability to mark critical checkpoints as "Out of Service." Checkpoints will provide a notification to the operator at the end of submitting an inspection if a Out of Service Failure was identified.  2) This will also provide status tracking on the resolution of the issue. 3) If an issue has been found on a compliance template, the operator will be required to mark the issue as Not Resolved, or sign off verifying that the repairs have been made.

For example, if you turn on the "Compliance Template" toggle and edit a component . . .

. . . you will now see that your "Checkbox" type questions have an additional toggle titled "Out of Service Failure."

If desired, toggle on "Create Issues on Checkpoints."

If this is enabled, when an issue is reported on an inspection, the issue will be created as the checkpoint rather than the component. For example, if a component is titled "Under the Hood" and your check points include "Oil Level", "Engine," and "Belts," this toggle would ensure that if "Belts" failed that the issue name would be Belts, rather then "Under the Hood."

The inspection checkbox failures will be found under the "Issues" tab.

If you desire to require inspections, select the "Inspection Required" toggle.

You can require an inspection by movement in or out of landmarks, by a fixed schedule (daily, weekly, monthly, etc.) or by ignition status on or off.

For example, if your fleet leaves from a company location each morning, you could require a pre-trip by movement out of that landmark.

Now, select which location(s) should be monitored for movement.

For example, if your company has multiple locations, you may want to require a pre-trip by "Landmark Category" to account for multiple locations.

Then, set a grace period for operators to complete their inspection.

A "Fixed Schedule" requirement will identify an inspection as missed if the Fixed Schedule date has passed without an inspection on record for the form's assigned asset.

An "Ignition On/Off" requirement would require an inspection to take place upon the ignition being turned on (for pre-trip) or off (for post-trips). Ignition status forms also allow grace periods.

Once you have made your selections for inspection requirements, select "Submit."

Now, select the "Operator Assignment Rule." Using this rule will allow you to automate the assignment or un-assignment of operators to vehicles based on the completion of inspections. This ensures that you are tracking operator trends and insights across the IntelliShift plaform.
